Insurance Information

1. RELEASE FROM LIABILITY: In consideration of the acceptance of my and/or my child’s application to be a participant in the Parks and Recreation programs/class (hereinafter referred to as “the program”), I hereby agree to assume all risks attendant upon myself and/or my child while participating in the programs.  I hereby waive, release, and discharge any and all claims for damages for death, personal injury or property damage which I or my child may have, or which may hereafter accrue to me or my child, as a result of my or my child’s participation in the program.  I agree to save and hold harmless from liability the City of Oceanside, all other city agencies and/or any of their agents, volunteers, or employees by reason of accident, death, injury or damages to persons or property, which my child or I may suffer while participating in the programs.  This release is intended to discharge in advance the City of Oceanside, all other city agencies and/or any of their agents, volunteers or employees by reason of any accident, death, injury or damages to persons or property which I or my child may suffer, from and against any and all liability arising out of or connected in any way with my child’s participation in the programs, even though that liability may arise out of negligence or carelessness on the part of the persons or entities mentioned above.

2. CODE OF CONDUCT: In consideration of the benefits of the program, my child and I agree to abide by the Code of Conduct set forth in this section. I understand that this Code of Conduct ensures the quality and safety of the program, and that all participants, staff, parents, spectators, coaches, and volunteers are required to abide by this Code. The Code of Conduct states that: “All persons shall act with respect toward others, observing their privacy and safety. Physical or verbal abuse of any kind will not be tolerated and will result in the participant being asked to leave the programs. All persons shall show respect to the property of others; intentional damage to, or theft of, such property or program equipment will result in the participant being asked to leave the program. Program rules and regulations shall be observed at all times. While staff is committed to having the participant enjoy the experience, inappropriate behavior may result in a participant being asked to leave.” No refunds will be given in the event that a participant is asked to leave the program for violating the Code of Conduct.

3. REFUND POLICY: I understand that the programs are self-funded. I therefore agree that a 20% per participant, per session, charge shall be assessed for cancellations made prior to 3:30 pm on the Thursday immediately before the beginning of sessions, and that no refunds will be issued thereafter.

4. VIDEO AND PHOTOGRAPH RELEASE: I understand and agree that the City of Oceanside reserves the right to take and use videos and photos of participants, free of charge, to be used for publicity purposes in the City’s program guide and/or other media publications.

5. CANCELLATION OF SESSION: I understand and agree that the City of Oceanside has reserved the right to cancel any or all scheduled sessions, and that should a session be cancelled, I will receive a full refund.

6. SWIM ASSESSMENT: I understand that prior to participating in aquatic activities, all participants will receive a swim test conducted by a certified lifeguard. The results of this test will determine the extent of participation in aquatic activities. Participants will be limited to areas within their ability.

7. TREATMENT AUTHORIZATION: I, the participant or the parent or legal guardian of the child listed above, do hereby authorize and consent to any X-ray, examination, anesthetic, medical, or surgical treatment rendered under the general or special supervision of any member of the medical staff and emergency room staff licensed under the provisions of the Medical Practice Act or a Dentist licensed under the provisions of the Dental Practice Act and on the staff of any acute general hospital or emergency care facility holding a current license to operate a hospital or emergency care facility from the State of California Department of Public Health.  I understand that this authorization is given in advance of any specific diagnosis, treatment or hospital care being required, but is given to provide authority and power to render care that a physician, in the exercise or his/her best judgment, may deem advisable for myself or for my child.  Further, I understand I or my child may be participating in inherently dangerous activities. I understand that all effort shall be made to contact me prior to rendering treatment to my child, but any of the above treatment will not be withheld if I cannot be reached.  This authorization is given pursuant to the provisions of the California Family Code Section 6910. 

8. DROP OFF/PICK UP POLICY: I understand that participants are to be dropped off no sooner than ten (10) minutes prior to the program start time, at the designated location for the program site/location, and that pick up is at the same location NO LATER THAN the program end time. For every five (5) minutes that a camper is picked up late $1.00 will be charged, and is to be paid at time of pick-up. Please see your receipt for detailed information on your program. I understand that unless I provide written instructions to the program supervisor limiting those persons who may pick up my child, my child may be released. I understand that before/after care is not available for all programs.

9.  CHILD ABUSE MANDATED REPORTERS: I understand that City Parks and Recreation Department staff are required by state law to report any suspected cases of child abuse or neglect to the appropriate authorities for investigation.

10. FIELD TRIP PERMISSION: I understand that part of the activities of many youth programs involves field trips that require walking to parks or public sites. Some trips will require transportation on public buses, trolleys, or City-approved vehicles. Staff walks with the group in a safe manner and campers will be supervised at all times. Further, I understand that if my child is not participating on the field trip, I must make other arrangements for child care.

11. BEHAVIORAL CONTRACTS: I understand that my child is expected to behave appropriately when at the program and on trips with the program. Inappropriate behavior may result in a child participant entering into a Behavioral Contract. The parent/guardian will be apprised of the reason(s) given of the need for the Behavioral Contract. Staff is committed to having the child participant remain with our program to enjoy the program experience. However, if that inappropriate behavior does not change, the child participant may be asked to leave the program.

12. COVID-19 GUIDELINES: On behalf of myself and/or my child, I understand and agree as follows:

  • I agree to follow Center of Disease Control (“CDC”), State/County guidelines and procedures for social distancing and health and safety to reduce the spread of Novel Coronavirus, or COVID-19. I understand that this will require me to wear a mask when within six (6) feet of other individuals outside my own household and to maintain as much distance as possible between myself, instructors, volunteers, employees and patrons of the organization. I agree to follow all of the guidelines set forth by the Oceanside Parks and Recreation Division.
  • I agree to utilize surgical masks or improvised masks such as scarves, bandanas, buffs and handkerchiefs to reduce the risk of exposure to myself and others.
  • I agree to wash my hands before participating in recreation programs.
  • I agree to participate in mandated sanitizing procedures after using equipment, and I agree to wash or sanitize my hands before and after activities, as well as after using the restroom, sneezing, or coughing.
  • I agree to follow all posted guidelines and restrictions of the CDC, State of California, and County of San Diego while participating in recreation programs.
